Used in both generalist and specialist settings, this tool can assist in matching the types and levels of need experienced by people with progressive chronic diseases (eg cancer, heart failure, COPD) and their caregivers with the most appropriate people or services to address those needs.

The PA Toolkit project was retired in June 2020, however this website includes links to alternate resources for general and specific palliative and aged care, together with practice resources for nurses.

There is increased awareness of the need for palliative care for chronic diseases. However, there remains a huge unmet need for palliative care for chronic life-limiting health problems in most parts of the world. The purpose of this Atlas is to shine a light on the need for palliative care globally and to provide useful information for those wishing to increase access.

This resource may help define appropriate primary palliative care and improve the quality of care delivered globally. Further studies are needed to evaluate their uptake and impact.
